Based on the notification from the Ministry of Health, Labour and Welfare, starting from September 2nd, 2024, we will be publishing the "Qualification Information Notice"(Password required for opening) on the "My Page" section.

 

The "Qualification Information Notice" is a notification issued to insured individuals, including dependents registered with Sony Health Insurance, to inform them of their qualification information, including the last four digits of their individual identification number. The purpose of this notification is to ensure the accuracy of the information and to allow everyone to use their My Number Card（Individual Identification Number Card）as their insurance card with peace of mind.

 

From the "My Page" of the insured individuals, including dependents covered by Sony Health Insurance, you can now view the qualification information. We recommend that you check and confirm if the information is correctly registered within the specified period.

 

Publication period    : September 2nd, to November 30th, 2024.
Target audience       : The insured individuals who are enrolled in Sony Health Insurance as of
　　　　　　　　　       August 18, 2024.
　　　　　　　　　       Dependents' information will also be displayed on the insured individuals
　　　　　　　　　       "My Page".
Opening password    : The password is mentioned in the body of topic '「資格情報のお知らせ」
　　　　　　　　　     （開封PWあり）をアップしました。' located at the top right of the 'My Page'
　　　　　　　　　       screen.

◆For individuals who enroll in Sony Health Insurance after August 19, 2024, we plan to send a notification without including the last four digits of their personal identification number around December 2024.

 

For those who have not completed registration for "My Page," we have sent a postcard requesting "My Page registration" in early August, 2024. Please register as requested.

 

※The "Qualification Information Notice" will not be published on "My Page" this time for those whose My Numbers have not been registered in our system for some reasons. We will contact them individually after September so that we can complete the registration. Please follow our instruction if you receive further notice. Your cooperation is highly appreciated.
