Childbirth

“A Childbirth and Childcare Lump-Sum Grant” is paid when an insured person gives birth. “A Dependents' Childbirth and Childcare Lump-Sum Grant” is paid when a dependent family member gives birth. You must undertake specific procedures to add a newborn child as a dependent under the Sony Health Insurance Society.

Request a Childbirth and Childcare Lump-Sum Grant

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (No prior procedures with the Health Insurance Society are required. For details, check with the medical care institution or other facility where you plan to give birth.)
Even if you use this system, you must apply to the Health Insurance Society to receive additional benefits.

Required documents:
Copies of documents on agreement to use the system from the medical care institution or other facility
(This must state that specifics such as the date of delivery, number of infants delivered, actual number of days hospitalized, amount received on your behalf, and the name of the medical care institution are consistent with the information on the claim form submitted to the Society and, in the case childbirth is conducted at an institution affiliated with the maternity medical care compensation scheme, must bear a stamp certifying that the delivery took place at a member institution.)
  • Copies of receipts for and details of childbirth expenses
    (Documents demonstrating that the details are the same as those on the special invoice submitted by the medical care institution or other facility to the Health Insurance Society and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)

  • Copies of documents related to agreement to use the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

    (indicating the name of the Health Insurance Society as application recipient)
    • * Must be attached in cases of childbirth within six months after dependent certification
Deadline: Two years from the day after the delivery date
Applies to: An insured person or dependent under the Sony Health Insurance Society at the time of childbirth

Notes:
  • Even if childbirth expenses are less than the amount of the Childbirth and Childcare Lump-Sum Grant to be paid, the amount paid will not change.
  • In the event of a stillbirth, documentation of the number of weeks in utero (for example, a copy of the “Birth Status” page of the maternal and child health handbook) must be attached as well.

If you paid the entire amount of childbirth expenses to the medical care institution

Submit the following application to the Health Insurance Society if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf.

Required documents:
  • Copies of agreements from the medical care institution or other facility
    (documents demonstrating that you have not concluded a proxy agreement with the medical care institution or other facility on use of the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions and indicating the name of the Health Insurance Society as application recipient)
  • Birth certificate (not required if certification was entered under item 18 on page 2 of the claim form)
    One of the following: birth certificate (original), Certificate of Acceptance of Birth Report (original), or official copy of part of family register (original)
  • Copies of receipts for and details of childbirth expenses
    (Documents demonstrating that the details are the same as those on the special invoice submitted by the medical care institution or other facility to the Health Insurance Society and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)

Notes: For stillbirths, obtain certification in space ⑱ (“Certification of physician or midwife”) on the second page of the request form.

When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Society in advance.

Required documents:
Application accepted You may apply as early as two months before the anticipated delivery date.
Applies to: Insured persons and dependents expecting childbirth up to two months in the future who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

Notes: Small birthing facilities must have submitted notices to the Ministry of Health, Labour and Welfare to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf. Confirm this with the facility in advance.

Childbirth occurring overseas

Required documents:
  • Birth certificate (not required if physician certification is entered under “Doctor’s Certification” on page 1 of the claim form)
    One of the following: birth certificate (original), Certificate of Acceptance of Birth Report (original), or official copy of part of family register (original)
Application accepted Two years from the day after the delivery date
Applies to: An insured person or dependent under the Sony Health Insurance Society at the time of childbirth

Notes: Documents attached to the claim form must be originals. (Documents may not be sent as email attachments, by fax, or by other such means.)

Add the child

After the child is born, take the necessary steps to add him or her as a dependent.
