When you incur high medical care costs

Certificate of Application of Maximum Copayment Amount

Required documents:
Applies to: Insured persons or dependents of less than 70 years of age who expect to pay more than their Cost-Sharing Maximum Amounts for medical care costs incurred at the medical care institution over one month

Notes:
  • You can use this system for both inpatient and outpatient costs.
  • Persons aged 70 or above can receive the same services by presenting their elderly benefits cards.
  • Those aged 70 and older can enjoy the same handling by presenting their elderly benefits cards. For this reason, they do not need to apply for this program.
  • The certificate is valid from the first date of the month in which the application was received by the Health Insurance Society. In principle, it will remain valid for six months.
  • Submit the original application form (this will not be accepted if submitted by fax or email attachment).
  • Return expired certificates to the Health Insurance Society. (Please submit a notice of loss if you lose the certificate.)
  • If you still need a certificate after your certificate expires, submit an application form for a new one.

Certificates Issued for Specific Disease Treatment

Required documents:
Deadline: As soon as possible
Applies to: Patients suffering from haemophilia, AIDS for which they are administered antiviral drugs, or chronic renal failure requiring artificial dialysis

Notes:
  • Before submitting the application form, have the “doctor’s opinion” space in the center of the form filled out at the medical care institution.
  • Submit the original application form (this will not be accepted if submitted by fax or email attachment).
  • A certificate will be issued, which will be valid from the first date of the month in which the application was received by the Health Insurance Society.
  • Submit the Application Form for Reissue of Certificate Issued for Specific Disease Treatment/Notice of Lost Certificate if you lose or damage the certificate.
