- * Once a year, the Sony Health Insurance Society includes a Groupwide Survey on Medical Assistance from Municipal Governments in the quarterly periodical HAIJII. Please submit the survey form even if you remain eligible, as before.
To prevent duplication of benefits, automatic payment of high-cost medical care benefits and additional benefits will be suspended if either of the following applies after submission of the Notice of Eligibility for Medical Assistance:
- You did not pay medical care costs at the hospital.
-
The costs you paid at the hospital were lower than your normal copayment rate
(e.g., if you paid only 500 yen per visit or your copayment was reduced from 30% to 10%).
You may need to apply to the Sony Health Insurance Society if automatic payment has been suspended and you made a payment at a medical care institution. Apply to the section in charge at your municipal government first (since use of public funding is prioritized). Submit the required documents if you are instructed to apply to the Sony Health Insurance Society.
