Medical assistance from municipal governments

Those with children of ages eligible to receive medical assistance for young children

Submit this application to receive High-Cost Medical Care Benefits and Additional Benefits if you were ineligible for medical assistance for young children due to an income limit and made a copayment exceeding 20,000 yen for medical care costs at the counter of a medical care institution.

If you are eligible for medical assistance, apply to the section in charge at your municipal government.
If the section in charge at your municipal government asks you to apply to the Sony Health Insurance Society, submit the required documents.

Required documents:
  • Copy of receipt
  • * If you are eligible for and have received medical assistance from your municipal government, a copy of a document indicating the amount of assistance received is also required.
Deadline: Within two years of the day following the date on which you paid medical care costs

Notes:
  • Apply separately by patient, month in which the examination or treatment was provided, and medical care institution (by outpatient/inpatient/dental category).
  • Costs not covered by insurance, such as inpatient meal costs and premium bed costs, are not covered.
  • Since these benefits will be processed after the invoice (rezept)from the medical care institution is received by the Health Insurance Society, payment will occur three months or longer after the month in which the examination or treatment was provided.
  • If the copayment you paid at the medical care institution was less than 20,000 yen, check with your municipality’s government office. (Copayments of less than 20,000 yen are not covered by these benefits.)

Assistance other than medical assistance for young children

Those receiving medical assistance for people with disabilities, assistance for single-parent households, etc.

Notify the Sony Health Insurance Society if you are eligible for a medical assistance program other than the program of medical assistance for young children.

Required documents: “Notice of Eligibility for Medical Assistance”
Copies of the front and back sides of the medical assistance card (beneficiary card) issued by your municipality

Notes:

You will need to submit a notice that your eligibility for medical assistance has ended in the event of loss of coverage due to relocation, reaching the maximum age, etc.

(Use the same form as for the Notice of Eligibility for Medical Assistance. Circle “Ended” in the form title.)
  • * Once a year, the Sony Health Insurance Society includes a Groupwide Survey on Medical Assistance from Municipal Governments in the quarterly periodical HAIJII. Please submit the survey form even if you remain eligible, as before.

To prevent duplication of benefits, automatic payment of high-cost medical care benefits and additional benefits will be suspended if either of the following applies after submission of the Notice of Eligibility for Medical Assistance:

  • You did not pay medical care costs at the hospital.
  • The costs you paid at the hospital were lower than your normal copayment rate

    (e.g., if you paid only 500 yen per visit or your copayment was reduced from 30% to 10%).

You may need to apply to the Sony Health Insurance Society if automatic payment has been suspended and you made a payment at a medical care institution. Apply to the section in charge at your municipal government first (since use of public funding is prioritized). Submit the required documents if you are instructed to apply to the Sony Health Insurance Society.
