If you have a valid Health Insurance Eligibility Certificate, submit the application form with the certificate attached.

- A family member who becomes an insured person under another health insurance plan; for example, he or she begins working
Copy of a document showing the date of joining social insurance (e.g., employment contract, notice of working conditions, letter of appointment [for public officials only], Notice of Eligibility Information, Health Insurance Eligibility Certificate)
- A family member who begins receiving employment insurance benefits
Copy of the certificate of eligibility for employment insurance benefits (all pages)
- * The dependent’s qualification will be canceled starting on the date on which he or she starts to receive payment of benefits (the day after the ending date of the waiting period, benefit restrictions, etc.), not the date on which he or she completed procedures at the Public Employment Security Office (processing date).

If you need a certificate of disqualification, check the box indicating that issuance of a dependent removal certificate is required on the Dependent Notice (Notice of Change) [Dependent Removal].
The certificate of disqualification will be issued to the insured person. It will be uploaded to the insured person’s My Page and, for the time being, also issued in paper form at the same time.
- “Certificate Issue Request”
A certificate of disqualification will be issued to the dependent if it is difficult to contact the insured person due to divorce or other reasons.
Fill out the necessary information on the Certificate Issue Request and submit it to the Sony Health Insurance Society with documentation of the dependent’s address attached.

Insured persons with dependents who meet any of the following:
- The family member becomes an insured person under another health insurance plan; for example, he or she begins working.
- The family member no longer meets any of the requirements for dependent certification.
(For example, his or her income exceeds the indicated income status; he or she begins to receive unemployment benefits under employment insurance; or he or she and the insured person divorce.)
- The dependent dies.
